This Privacy Policy explains how Ayul Community Care collects, uses, stores, discloses and protects personal information obtained through our website, contact forms, referral forms and during the delivery of disability support services. This policy applies to participants, prospective participants, family members, nominees, guardians, carers, referrers, employees, contractors, job applicants, website visitors and any other individual whose personal information we collect.

3. Information we collect

We may collect personal information such as names, addresses, telephone numbers, email addresses, dates of birth, emergency contacts, NDIS participant numbers, funding details, support needs, goals, medical and health information, allied health reports, referral information, payment information, feedback, complaints and correspondence. Through our website we may also collect technical information such as IP address, browser type, operating system, pages viewed and approximate location.

4. Sensitive information

As an NDIS provider we may collect sensitive information including disability, health, cultural, language and support information where reasonably necessary to provide services or where required by law. Sensitive information is handled with additional care and confidentiality.

We collect information directly from you, your authorised representative, healthcare professionals, support coordinators, plan managers, the NDIA, government agencies, referral partners, and through our website’s contact and referral forms. We may also collect website analytics through cookies and similar technologies.

We collect information to respond to enquiries, assess referrals, deliver supports, develop service agreements, communicate with participants, manage invoicing, comply with legal and NDIS requirements, improve our services, protect health and safety, investigate complaints and operate our business.

Recipients may include the NDIA, plan managers, support coordinators, healthcare professionals, emergency services, insurers, auditors, IT providers and regulators.

We take reasonable physical, technical and organisational measures to safeguard information against misuse, interference, loss, unauthorised access, modification or disclosure. Access is restricted to authorised personnel and contractors who require the information to perform their duties.
